This unit will provide you with the foundational skills needed for effective clinical case management, with respect to a range of clinical conditions and diseases. The emphasis and focus of the learning opportunity will be on providing person centred care through dietetic education and patient case management. You will be provided with opportunities to learn through simulation and case-based learning platforms. Further to this, the key focus of this unit is to ensure that you are able to demonstrate professional competencies as outlined by the Dietitians Association of Australia.

Learning outcomes
- Evaluate the role of diet in the aetiology, prevention, and management of a range of clinical conditions and diseases.
- Integrate knowledge of science and disease into nutrition assessment and management of individual cases including consideration of broader issues regarding healthcare systems and provision, management, ethics and quality improvement.
- Formulate effective individual case management plans (including prioritising care and discharge planning), drawing on core skills in the dietetic process.
- Integrate personal, social, clinical, cultural, physiological, ethical, environmental and economic factors into clinical reasoning and decision making in individual patient case management.
- Critically analyse and apply evidence-based approaches to individual patient case management
- Integrate theories into counselling and education, including behaviour change, to support effective person-centred individual patient case management.
- Apply a range of methods and strategies to communicate effectively, clearly and concisely to diverse audiences, adapting or co-creating communication messages for specific audiences across areas of practice
Workload
This unit will run over 7 weeks with an average of 22 hours per week allocated including own time and in class study as outlined below. 8hrs per week recorded lectures, reading and own time activities (pre-class) = 56hrs total 10hrs per week (5 x 2hrs) workshops on campus (in class) = 70hrs total 3 x 8hr workshops (full days) off campus (public health hospitals) = 24hrs total 1 x 2hr tutorial (interprofessional learning) on campus = 2hrs total
