The Disability Services takes into consideration the impact of your disability in the academic environment. If you require additional support and advice, it is vital that you register with the Disability Services as soon as possible.

  • You are required to fill up this registration form.The registration form needs to be completed, signed, and dated by both a health professional (Part A) and the student seeking registration (Part B and Part C).You may provide supporting medical documents (if any), and your supporting medical documents should:Email the PDF form and supporting documents (if any) to disabilitysupportservices@monash.edu
  • A DSS adviser will get in touch with you to talk about any adjustments you may need for your learning. You can talk to them about this by email, on the phone, or through virtual conversation via Zoom.Your registration with DSS and all the discussions you have with disability advisers are private – no information will be released or shared without your permission. Also, your registration with DSS does not appear on your academic record.
  • You will receive an email confirmation of registration if you're eligible for DSS arrangements and, if so, what they are.Services and support will be provided as discussed and agreed.

Any sensitive information you disclose is kept securely within Disability Services. Certain specific information about you may be disclosed to other areas of the University with your consent in order to arrange relevant services for you. If you have concerns about privacy, ask the Disability Services officer about their obligations to keep your information private.
