Overview

In this unit, you will develop advanced capabilities in the assessment and management of people living with chronic, complex and life-limiting illness in community, primary care and urgent-care settings. You will consider how multimorbidity, frailty, polypharmacy, functional status and psychosocial circumstances influence clinical presentation, decision-making and patient outcomes. You will apply advanced clinical assessment, diagnostic reasoning and therapeutic medication-management principles to conditions including cancer, renal and endocrine disease, diabetes, heart failure, chronic respiratory disease and dermatological presentations. You will also explore the care of older adults, palliative and end-of-life care, advance care planning, interprofessional coordination, referral, escalation and safety-netting. Through online learning, clinical cases, tutorials and facilitated discussion, you will formulate and justify person-centred management plans incorporating pharmacological, non-pharmacological, preventive and supportive care.
